  6. at each of the update points.  Revisions left by other means
  7. will not be incorporated into the list.  This assures that
  8. the list contains complete, accurate information.

  10. When leaving information, be sure to COMPLETELY fill out the
  11. questionnaire even if you are only changing certain
  12. information. This allows the editor to verify current
  13. information. NOTE: Please include the (OLD) PHONE NUMBER 
  14. on the list at a minimum for updates so I can find the 
  15. old entry.

  63. locations.  "xxx" is the version number.

  65. In most cases, revisions are completed the last weekend of 
  66. the month for the next month unless the first falls on a 
  67. weekend.  New versions are usually available sometime Sunday.
